To help us share these meaningful experiences through photos and videos on our website, social media, and promotional materials, we ask parents and guardians to complete this Photo Release Form. Your permission allows us to celebrate our ministries while keeping your child’s safety and privacy a top priority.

I give permission to Cave Spring United Methodist Church for the follow:
Use of my child’s image, by means of digital or film photography with respect to all Cave Spring United Methodist Next Generation Ministries activities and events.
- Use of my child’s image in any printed publications from the church.
- Use of my child’s image in any electronic publication from the church.
- Use of my child’s image on any social media page created for or by the church.
- Use of my child’s image on any website created for or by the church.
By signing this agreement on behalf of a minor child, I warrant that I am the legal parent or guardian of the child and have full legal authority to sign this agreement on behalf of the child.
